Avoiding Unplanned Admissions

From April 2014 the Practice has been reviewing any avoidable hospital admissions, this is designed by improving services for vulnerable patients and those with complex needs.

This also includes these patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them. This does not prevent you from seeing any GP in the practice as you currently do.

If you have information that you feel may be useful to our team (i.e. a carer’s, next of kin, keyworker details etc) then please would you be kind enough to contact the surgery by phone or letter and pass this information to our staff.
