Social Prescribing
Sometimes the things affecting your health are not medical, such as loneliness, money worries, housing, or losing confidence. Social prescribing connects you with a social prescriber who takes the time to understand what matters to you, and helps you find support in your community.

Who it can help
Social prescribing may help if you are:
- feeling lonely or isolated
- struggling with benefits, housing or debt
- experiencing mild anxiety, stress or low mood
- looking to get more active, to volunteer or to meet people
- caring for someone else
- finding it hard to get back to activities you used to enjoy
What to expect
Your social prescriber will meet with you, usually over a few sessions, to talk through what matters to you and agree a simple plan together. They can connect you with local groups, services and activities, and help with practical things like sorting out money, housing or benefits. It is not a quick fix, but many people find it makes a real difference.
How to get social prescribing
As well as asking us yourself, a GP, nurse or other clinician here can refer you.
Our social prescribing link worker is provided through our Primary Care Network by Together Co, a Brighton and Hove charity. If you prefer, you can contact Together Co yourself on 01273 775 888 or at togetherco.org.uk. Their service is for residents of Brighton and Hove aged 18 and over.
